It's harder. Poorer people will break more things, ask for more stuff, require more intensive one-on-one service, as a matter of objective reality. (2) What would be the price of a life saving vaccine if people would "charge what it's worth"? What about "what they could get"?

Okay. So you got it 200% wrong. We charge what we could get for life saving vaccines, and for that matter most life saving generics, which is very close to $0, even though people would probably pay a ton of money for it.
