Wouldn't it be possible to replace the internal battery while bridging it with an external power source? How would the device know, that the battery was exchanged?

It's possible when the battery is still working. When it's dead, replacing it for new won't fix anything.

Yes, I understand. But I seriously consider doing this as a preemptive provision.
