Great discussion on the importance of refactoring your design and code after making a series of isolated optimizations.
Countries like Israel and Estonia are doing something similar. Meanwhile in the US we are teaching only 10% of our schoolchildren how to code. It will be interesting to see what these countries will be producing in 20 years; but, what I am more worried about if the US will still have its technological prowess.
In the conclusion the allude to two areas for improvement: access and prevention.

1) "Our results on neonatal mortality strongly suggest that differential access to technology-intensive medical care provided shortly after birth is unlikely to explain the US IMR disadvantage"

2) "... in general, policy attention should focus on either preventing preterm births or on reducing postneonatal mortality."

ER Resident Physician here, we are many years in from The Joint Commission's administrators claiming that pain is the "5th vital sign" and mow feeling the effects of this disastrous policy. My ED is filled with drug seekers; studies have shown up to 20% ED visits may be Drug-Seeking Behavior (DSB). Really demoralizes physicians who want to help not only these people but those in the overflowing waiting rooms.
Both companies are hemorrhaging cash by operating in China. Its a game of who can survive longer, capital infusions are a way to lengthen the runway. Seems like an ugly battle.
