4 is a great idea. It would probably make us healthier.
Apart from that, we simply do not have enough data. What I'd like to do is repeat the RAND experiment. Take a large group of people at random, and give them different amounts/types of health care. Measure their health outcomes.
This will at least give us some data to make a more informed choice.
