If you have the time to mess around with Asterisk, it's doable on your own. That's a sizable "if" though.

There are lots of SIP phone services. I use sipgate which covers many countries and have an old VoIP phone or use my mobile.

I ended up using Yate with a hard phone to achieve this.

It works mostly, but far from trouble free.
