Some people don't wanna get promoted because its too much of the "Wrong Kind of work". (TM) ;). e:g less coding, more management BS, corporate stuff, playing silly games, jumping through pointless hoops, being forced to show off to others, pushing others to do things they don't want to and that are pointless.

This is absolutely me - I write code because I find it fun and it solves my problems, and then I have more fun solving other peoples problems, and omg wow they pay me for it! The vicious cycle won't end until I run out of things to build or (more likely) the new things to build are no longer fun.
I wonder if this is really just the key reason here. Perhaps everything else here just revolves around that. e.g. people being defensive with feedback and not analyzing perf reviews because they don't actually want to grow.
