+ both space shuttle crashes should not have happened. There were people involved in both circumstances ringing the alarm bell and warning of the likelihood of the outcome. Those were not failure of technology, they were failure of culture and courage (though in the case of Columbia, I'm not sure what the rescue plan would have been).
