I don't think book creator grabs history pages.

You can add them manually by going to each one and adding it to the book. I'm not sure if there is a less tedious way of doing it, though.

I suppose they could have been, but looking at the author's website it seems he runs an on-demand print house himself (whether he uses a third party provider to do the actual printing isn't clear).

I too would be interested in purchasing the(these) book(s).
