With programming/shell using Dvorak I experienced pain noticed most in pinky-heavy commands (e.g. ls<RETURN>).

I now map right alt to return and that's helped:

   xmodmap ~/.Xmodmap
in ~/.Xmodmap:

    ! 108 = Right Alt
    keycode 108 = Return
